Clinical pictures,treatments,and resource use of norovirus gastroenteritis in long-term care facilities: a survey with a chart review in Japan

概要

［背景］
　ノロウイルス感染性胃腸炎は、高齢者長期ケア施設(以下、施設)で集団発生(アウトブレイク)が散発すると、入所者が最も注意すべき感染症の一つである。ノロウイルス感染性胃腸炎は、日本の施設では臨床経過や周囲の感染状況から総合的に診断されることが多く、特異的な治療がない。日本の施設入所者へのノロウイルス感染性胃腸炎のケアの実情を知る公的な仕組みはなく、実情は不明である。そこで、日本の施設でのノロウイルス感染性胃腸炎の発生、臨床像と治療内容、治療に必要な医療資源を明らかにすることを目的に本研究を実施した。

［方法］
　調査協力施設への質問紙調査と、それらの施設の中で対象期間にノロウイルス感染性胃腸炎が発生した施設への診療記録調査である。日本の大阪府と京都府の19施設の2009年から2011年のノロウイルス感染性胃腸炎の発生状況を質問紙調査した。さらに、診療記録から感染者個々の特性、症状、治療内容、診断キットの使用状況、隔離状況、転帰を収集した。感染者一人の全薬剤費は、薬価と一日投与量と投与日数の積を1薬剤費とし、投与された全薬剤のそれぞれの費用の総和とした(USD$1を100円に換算)。

［結果］
　質問紙調査に回答を得た19施設において、3年間で6施設から8事例のノロウイルス感染性胃腸炎の集団発生(アウトブレイク)が確認された。診療記録閲覧の許可を得られた5施設107人の感染者の特性は女性86人(81%)、平均年齢85.4歳、有症期間は平均4日で、106人(99%)が嘔吐または下痢、84人(79%)が嘔吐・下痢以外の随伴症状(食欲低下、37.0度以上の発熱、全身倦怠感など)を呈した。診断は、32人(30%)が診断キット、75人(70%)が症状のみでなされていた。患者の隔離は、感染者数の50%から100%まで施設間でばらつきがあった。81人(76%)が投薬治療を受けており、主な治療内容は、点滴維持液67人(63%)、抗生剤30人(28%)であった。投与された全薬剤の費用は中央値USD$4.4、感染者一人一日あたりの費用は中央値USD$2.0、薬剤別投与全期間の費用は抗生剤がUSD$8.6で最も高かった。99人(93%)が施設で軽快、1人(1%)が施設で死亡、7人(6%)が転院した。

［考察］
　本研究は複数施設で集団発生したノロウイルス感染性胃腸炎の臨床経過を診療記録で確認したアジア圏からの第一報であり、これまで不明であった施設でのノロウイルス感染性胃腸炎の治療実態の一端を明らかにした。感染者107人の有症期間の代表値、症状の内訳は、米国の先行研究(7施設での集団発生)の臨床経過と類似していた。本研究で得られた薬剤費は安価であったが、抗生剤の不適切な与薬の可能性があるなら、少額の支出とはいえ投薬の見直しが示唆される。本研究では、各施設が感染者として対応した者を患者と定義したため、診断基準に施設間のばらつきがあり、感染者数が過大または過小見積もりされた可能性がある。また、協力施設は感染対策への意識が比較的高い集団と考えられるため、他の施設への一般化には慎重を要する。

［結論］
　高齢者長期ケア施設団発生したノロウイルス感染による胃腸炎患者の99%が嘔吐・下痢症状を呈し、有症期間は平均4日だった。約75%の感染者に投薬治療が行われ、点滴維持液、抗生物質が多く、薬剤費用は抗生物質が最も高かった。薬剤費用は安価であったが、投薬の適切性は問われるべき課題であろう。
