期肺癌は化学放射線治療により長期生存も期待できるようになった。しかし、多様性のあるⅢ期肺癌の中には外科手術の恩恵を受ける症例も存在する。今回、導入化学放射線療法後に外科切除し得た肺尖部椎体浸潤癌の1症例を経験したため報告する。症例は60歳代男性で、左肺尖部腺癌c-T4:Th2/3椎体浸潤NOMO,StageⅢAに対し、導入化学放射線療法としてシスプラチン(CDDP)十TS-1+同時胸部放射線照射(40Gy)を施行したところ、一定の縮小効果が得られたため、左肺S1+2区域切除術+胸壁合併切除(第2,3肋骨切除)を行った。病理病期はyp-T3【肋骨浸潤】NOMO,StageⅡBであり、切除断端は腫瘍陰性であった。Ⅲ期肺癌に対する治療成績向上を目指すには、各科の連携による集学的治療が必要であると考えられた。
