同時性両側多発肺癌の臨床像と治療成績につき自験例をもとに後方視的に検討した。2015年以降に当科で手術を施行した原発性肺癌284例のうち、画像的に同時性両側多発肺癌と診断され、両側に局所治療介入を行った5例を対象とした。治療順序は同時手術が1例、右先行が2例、左先行が2例で、治療選択は肺葉切除(以下葉切)+肺部分切除(以下部切)が1例、葉切+肺区域切除(以下区切〉が2例、葉切+部切が1例、区切+定位放射線治療(stereotactic adiotherapy:以下SRT)が1例、部切+SRTが1例であった。2019年12月現在、全例無再発または無増悪生存中である。両側性同時多発肺癌の治療に対しては、各症例の全身状態、患者背景に応じた慎重な対応が求められるが、適切な時期に治療が導入できれば良好な成績が得られる可能性が示唆された。
