パーキンソン病患者に対する訪問看護ケアの進行度ごとの差‐Hoehn & Yahr ステージIII, IV, V-

概要

【背景】人口の高齢化に伴い、・多くの国でパーキンソン病(Parkinson's Disease: PD)患者数が増加する中、今後のPD患者に対する看護は病院中心のケアモデルよりもホームベースのケアモデルが推奨されている(Dorsey,2016)。患者を支えるためにはエビデンスに基づく統一された訪問看護ケアが重要である。現在この分野の実態調査やガイドラインの作成が進んでいるが、どのようなPD患者にどのような訪問看護ケアが必要とされ実践されているか情報は十分でない(Axelrod, 2010, Lennarts, 2017)。

【目的】本研究は、PD患者に対して看護師が行う訪問看護ケアの違いを、患者のHoehn & Yahr(HY)ステージ(Unified Parkinson's Disease Rating Scale: UPDRSと同一のステージスケール）ごとに明らかにし、今後の看護ケアのプロトコル作成に示唆を得ることを目的とした。

【研究方法】2019年7月から12月、PD看護を専門とする訪問看護事業所が訪問中のPD患者と、看護を行う看護師を対象に調査を行った。同意を得たPD患者の訪問看護報告書・訪問看護記録・訪問看護指示書の記録と、自宅（又は施設）への看護師訪問時の看護ケア実施の観察記録を取得した。観察記録は研究者が訪問場面に同行して、看護ケア項目を作成しながらケア実施の有無を記録して取得した。

分析は、患者の属性、居住場所、同居人数、合併症、1月あたりの訪問回数をHYステージ間で比較した。また、親察記録から得た看護ケア項目の実施の有無を、HYステージ間で比較した。数値の比較はKruskal・Walli:s Testを、名義尺度の比較はFisher Exact Testを行った。統計解析はSPSS Statistics 26 (Windows10)を用い有意水準は5％とした。

研究は神戸大学大学院保健学倫理委員会の承認を得て実施した（第821号）。

【結果】21人のPD患者はHYステージmが7人、IVが8人、Vが6人だった。ステージIとIIの患者はいなかった。ステージ間で年齢と性別に有意差は無かったが、1月あたりの訪問看護サービス利用回数の中央値は、ステージrnが8.0日、IVが11.0日、Vが17.5日で有意差があった(p<0.01)。居住場所では、ステージrnは全員自宅で過ごし、IVは施設で過ごす人が増え、有意差があった(p<0.05)。同居人数の中央値は、ステージIIIが3.0人、IVが1.0人、Vが1.5人で、有意差があった(p<0.05)。全員が公的医療助成に関する特定疾患医療受給者証を所持していた。

21人の24回の訪問看護場面を槻察して作成された看護ケア項目は29項目だった。全てのHYステージで実施割合が多かった看護ケア項目は計測・親察(100%）、患者への相談・教育(76.2%）、与薬、マッサージ・四肢の他動運動、準備・片付け（各52.4%）、水分摂取、家族への相談・教育、連携・記録（各42.9%）、だった。ケア項目のうち、オムツ交換・陰部洗浄、摘便・浣腸、導尿・留置カテーテルのケア、患者への相談・教育を実施した割合にHYステージ間の有意差があった(p<0.1)。

【考察】結果から、各ステージのPD患者への看護は以下のものだと考えられた。HYステージIIIでは、自宅で療養中の患者に週に2回程度訪問するのが標準的である。薬の定期的な自己服用を助けるため配薬や本人・家族への相談など、自宅で自立して過ごすことを間接的に助ける看護ケアが一般的である。ステージIVでは、施設で過ごす患者が増えるが、週に3回程度訪問するのが標準的である。直接的な身体ケアも増え始め、関節や筋肉の痛みを和らげる四肢のマッサージなど緩和のケアも多い。ステージVでも、自宅で過ごす患者もいる。週に4回程度訪問するのが標準的である。ベッドで過ごす時間が増え、褥癒ケア、オムツ交換や整容のケアも急速に必要になる。看護師が与薬することも増え、直接的な身体ケアが中心となる。緩和のケアや本人への相談は減っていた。

【結論】本研究においてHYステージrrr、IV、Vの間で看護ケアに明らかな違いが観察された。本研究で得られた訪問看護の頻度と性質に関するステージをベースとしたプロトコルが、PD患者への一貫した効果的な看護ケアを確保するのに役立つ可能性が示唆された。
