Dynapenia and Sarcopenia Associated with Abnormal Renal Function in Chinese Elderly Community with Impaired Fasting Glucose and Type 2 Diabetes

Aims: The aim of the present study is to determine whether there is relationship between sarcopenia or dynapenia and abnormal renal function in pre-diabetes and diabetes among community older adults.

Materials and methods: Our study population comprised of six hundred and forty community elderly (mean ± SD age: 68.0±6.1 years; 57.0% women) from Tianjin, China. Pre-diabetes is defined by having fasting plasma glucose (FPG) levels ≥100mg/dl (5.6mmol/L) but <126mg/dl (7.0mmol/L); diabetes is defined by self-reported or FPG≥126mg/dl (7.0mmol/L). Sarcopenia is defined by the definition of Asian Working Group for Sarcopenia (2019 Consensus Update); dynapenia is defined by low grip strength ( <28kg for men and <18kg for women) and low knee extension strength ( <1.13Nm/kg for men and 1.01 Nm/kg for women).

Results: After multivariate adjustments for all the concomitants, including sex, age, body composition variables (BMI, WHR, T value, % body fat, visceral fat area), lifestyle-variables (history of fall, physical activity, nutritional status and emotion status), health status variables (TC, TG, hypertension, dyslipidemia, stoke, CHD, duration of diabetes, medication, sarcopenia or dynapenia), we found that the lowest quarter of eGFR was associated with dynapenia in subjects with pre-diabetes (OR 4.70, P=0.039) and diabetes (OR 9.19, P=0.036). However, we did not find the relationship between sarcopenia and renal function in pre-diabetes and diabetes.

Conclusions: The present study indicates that a positive relationship among dynapenia and renal dysfunction in pre-diabetes and diabetes.This result may indicate that assessment of muscle functionality may provide additional diagnostic and prognostic information about renal function to muscle-mass evaluation among community elderly with pre-diabetes and diabetes.
