乳がんサバイバーの健康課題解決に向けた包括的検討 : 体力、身体活動量、体重変化に着目して

概要

乳がんは日本人女性の中で最も罹患数の多いがんであり、2015年には約9万人が乳がんと診断された(国立がん研究センター, 2020b)。国立がん研究センターによると、乳がん罹患者数は2015–2019年の平均が89, 850人だが、2030–2034年の平均が113, 930人と今後も乳がん罹患者数が増加する見込みである(国立がん研究センター, 2020b)。乳がんの5年後生存率は92.3%と他のがん部位と比べて高く、乳がん治療後は多くの乳がんサバイバーが社会復帰していく。

 乳がんサバイバーは乳がん治療に伴って様々な健康課題が指摘されており、心血管疾患、体力低下、体重増加、倦怠感等が報告されてきた(Carreira et al., 2018; Gernaat et al., 2018; Lakoski et al., 2013)。乳がんサバイバーの心血管疾患発症リスクは乳がんと診断されていない同世代の健常女性よりも高く(OR[odds ratio]: 1.44; 95%信頼区間[95% confidence interval: 95% CI]: 1.00–2.06)(Gernaat et al., 2018)、心血管疾患死亡リスクも同世代の健常女性より有意に高いことが報告されている(OR: 1.80; 95%CI: 1.30–2.50)(Bradshaw et al., 2016)。乳がんサバイバーの心血管疾患が多い理由の1つとして化学療法に伴う心毒性が指摘されており、Klassen et al.(2014)は化学療法を受けていない乳がんサバイバーの最高酸素摂取量は19.8±5.4mL/kg/minであったのに対し、化学療法後の乳がんサバイバーの最高酸素摂取量は15.5±4.8mL/kg/minと有意に低かったと報告した。Peel et al.(2014)の総説によれば、補助療法(手術、化学療法、放射線療法)後の乳がんサバイバーの最大酸素摂取量は22.2mL/kg/minで、乳がんと診断されていない健常女性の29.7mL/kg/minよりも有意に低かったと報告した。一般的に心肺持久力は心血管死亡リスクと負の関連があり、乳がんサバイバーにおける心肺持久力の低下は長期的に乳がん予後に悪影響を与える可能性がある(Kodama et al., 2009)。また、乳がんサバイバーの肩関節可動域も低値であることが報告されており、Harrington et al.(2011)は乳がん手術、化学療法を終了した乳がんサバイバーの肩関節屈曲角度は、健常女性よりも有意に低いことを報告した。乳がんサバイバーは心肺持久力や柔軟性が低く、日常生活に支障をきたしている可能性が考えらえる。

 乳がんサバイバーは乳がん診断後に体重が増加することも多くの研究で報告されており、Irwin et al.(2005)は乳がん診断から3年以内に、68%の乳がんサバイバーで体重増加があったと報告している。Kroenke et al.(2005)はBMI2kg/m2以上の過度な体重増加は乳がん再発と関連したと報告しており、乳がん再発を予防するために過度な体重増加を防ぐことは重要である。その他の健康課題として倦怠感の上昇も指摘されており(Jacobsen et al., 2007)、様々な副作用の影響を受けて乳がんサバイバーの生活の質(quality of life: QoL)の低下が懸念されている(Carreira et al., 2020)。上記に記載した乳がんサバイバーの健康課題の解決に向けて、適度な運動実践および食習慣改善は効果が期待でき、欧米では複数のランダム化比較試験(randomized control trial: RCT)がおこなわれてきた。Scott et al.(2020)は174人の乳がんサバイバーを3群(週160分の運動群、週120分未満の運動群、対照群)にランダムに割り振り、最大酸素摂取量の変化を調査した結果、運動群は最高酸素摂取量が有意に上昇したが、対照群で有意な変化は認められなかった。Goodwin et al.(2014)は338人の乳がんサバイバーを運動および食事介入群と対照群にランダムに割り振り減量効果を調査した結果、6ヵ月後の介入群の体重減少率は5.3±4.9%、対照群は0.7±4.9%で有意な交互作用が認められ、介入群の身体的QoLの改善も認められた。このように、乳がんサバイバーにおいて運動実践および食習慣改善は健康課題の解決に効果的な可能性があるが、日本では乳がんサバイバーを対象とした運動や食事の介入研究は非常に少ない。その理由として、日本と海外では診療報酬システムが異なり(Mewes et al., 2015)、乳がんサバイバーを対象とした運動指導の経済的インセンティブが少ないことや、日本における乳がんサバイバー向けの運動は比較的新しい分野であることが挙げられる。欧米と日本では乳がんサバイバーの体格や生活習慣が異なる(WHO Expert Consultation, 2004)ことを考慮すると、日本人乳がんサバイバーを対象に、運動実践および食習慣改善が乳がんサバイバー特有の健康課題にどのような影響があるかを検討することが必要だと考えられる。
