胃癌治療における周術期支持療法の確立 ─集学的治療における体組成変化の影響と対策

概要

消化器癌を治癒するためには根治的な切除が必要不可欠である.しかしながら,局所だけに留まる早期の固形癌を除き,肉眼的な治癒切除を達成したとしても再発することが少なくない.再発する消化器癌では,手術時に遠隔臓器に微小な転移を来していると推測されている.この微小転移の根治をめざした治療が補助療法である.ある程度進行した胃癌,大腸癌,乳癌,肺癌,食道癌,膵臓癌など多くの固形癌では,外科的な根治切除+術前または術後の補助療法よりなる集学的治療が標準治療となっている.一般に,高度な生体侵襲を伴う外科切除では,異化が亢進した体重減少や筋肉量減少などの体組成変化が生じる.食事摂取量が低下しやすい化学療法においても異化が亢進し体組成変化が生じる.このため消化器癌の集学的治療中には,体組成が大きく変化する.これまでの報告から様々な癌種において集学的治療中の体組成変化は,癌集学的治療の有害事象発現や治療継続性との関連に影響する重要な因子の一つであることがわかってきた.さらに,体組成変化は術後合併症の発生,予後にも影響することがわかってきた.周術期の体組成変化の影響とその課題を報告する.また,現在集学的治療における体組成変化に対して栄養療法,免疫栄養療法,運動療法などの対策がすすんでいるが,これらの対策の現状と課題に関しても報告する.
