(地域在住高齢者における口腔の健康、QOL と包括的ヘルスリテラシー
との関連性)























全身の健康に影響するため、口腔内の健康を良好に保つ重要性が認識されている。また、口腔疾
患の有病率にはヘルスリテラシーの低さが関連している。そこで、本研究の目的は、地域在住高齢
者の包括的ヘルスリテラシーが客観的な口腔衛生および口腔健康関連 QoL(OHRQoL)と関連する
かどうかを明らかにすることとした。
65 歳以上の参加者へ自記式質問票を配布した。同日、口腔健康評価ツール(Oral Health Assessment
Tool)で収集したデータを用いて、参加者の口腔状態を客観的に評価した。質問票には、OHRQoL
を 測 定 す る た め の General Oral Health Assessment Index と 包 括 的 ヘ ル ス リ テ ラ シ ー を 評 価 す る
European Health Literacy Survey Questionnaire の短縮版を含めた。データは単変量および多重ロジス
ティック回帰法により解析した。
本研究に参加同意したのは 145 名で、うち 118 名が本調査の有効回答者(81.4%)となった。118
名のうち、客観的口腔衛生の「口腔清掃」が「病的」であったのは 18%であった。多重ロジスティ
ック回帰分析により、包括的ヘルスリテラシーは口腔清掃と OHRQoL の関連因子として特定された
(各々OR=5.00;3.33, p<0.01; p<0.05)。
これらの結果から、包括的なヘルスリテラシーが、臨床上の成果を変えることを示唆している。
高齢者は口腔の健康問題だけでなく、併存疾患を抱えていることが多いため、看護師は併存疾患の
フォローアップの機会に、ヘルスリテラシーを評価し、個人に合わせた口腔健康指導を行い、
OHRQoL を向上させていくことが重要である。
