Non-motor symptoms depending on motor severity in Japanese patients with Parkinson's disease: A multicenter cross-sectional study

概要

パーキンソン病患者の診断や重症度分類は主に運動症状を中心に行われてきた。近年、非運動症状はパーキンソン病患者のQuality of Life の低下に与える影響が大きいとの報告から注目され、非運動症状への適切なアプローチが重要とされている。しかし非動症状は主観的で、医療者は患者の症状を把握するための問診や質問紙を利用するなどの取り組みが必要となり、臨床現場で見過ごされることが多いことも指摘されている。

多くの研究で非運動症状の有無や症状の悪化と運動症状の重症度との関連が示唆されている。一方、非運動症状は年齢・性別・罹病期間など運動症状以外の要因との関連も報告されているものの、非運動症状と運動症状の関連について他の要因が考慮されていない研究も多い。本研究ではパーキンソン病患者の非運動症状の有無は運動症状の重症度に依存しているのかを明らかにするため、非運動症状に関連する他の要因を加味した分析を行った。

日本の7施設が参加したパーキンソン病患者レジストリーに登録された患者のうちDeep Brain Stimulationを受けた患者を除いた1037人を対象とした。非運動症状の有無を目的変数、運動症状の重症度(Hoehn & Yahr stage:HY stage)を説明変数とし、非運動症状に影響すると考えられている要因(年齢、性別、罹病期間、ウェアリングオフの有無、ジスキネジアの有無、臨床サブタイプ[振戦優位、中間、姿勢反射障害・歩行障害]、 L-ドパ換算用量相当量)を調整変数としたロジスティック回帰分析を行った。非運動症状の有無についてはNon-motor symptoms Scale(NMSS)を使用した。

NMSS のドメイン別の解析では D1(心血管系)、D2(睡眠/疲労感)、D3 (気分/認知)、D4(知覚障害/幻覚)、D5(注意/記憶)、D6(消化器系)において非運動症状有りと運動症状の重症度に関連がみられた(Odds Ratios [ORs]: 1.54-2.72, いずれもP<0.05)。しかし D7(排尿)、D8(性機能)の症状有りと運動症状の重症度との関連は見られなかった。NMSSのアイテム別の解析では Item 2(失神)と Item 14(妄想)の症状有りと運動症状の重症群(HY stage 4-5)に強い関連がみられた(ORs: 9.95 と5.92, P<0.05)。

運動症状の重症度に関連が見られた非運動症状に関しては、運動症状の重症度は非運動症状の存在を予測する指標になるだろう。また運動症状の重症度と関連が見られなかった症状に関しては、より影響を及ぼす可能性のある他の要因の検討が必要である。これらの結果はパーキンソン病患者の非運動症状に対する病態の理解や、臨床現場での非運動症状に対する取り組みを改善する一助となるだろう。
