足関節不安定性を伴う姿勢制御能力の評価法

概要

足関節捻挫は,最も頻繁に発生するスポーツ傷害である1,2.例えば,Fongら3によると,227件の疫学調査(70種のスポーツ競技,201,600名の対象者を含む)をレビューした結果,70種類中24種(34%)の競技において,足関節は最も共通した損傷部位であったことが報告された.また,Dohertyら4による足関節捻挫発生状況の系統的レビューの結果,女性における発生率が男性よりも高いことが示された(女性:13.6/1000exposures,男性:6.94/1000exposures).また,発生率は年代別で異なっていた(12歳未満のアスリート:2.85/1000exposures,12から18歳の青年アスリート:1.94/1000exposures,19歳以上の成人アスリート:0.72/1000exposures).両報告において,スポーツ傷害の中でも足関節捻挫と診断される足関節損傷が大半であるという見解は一致しており,さらに,その80%から90%は足関節外側捻挫であったことが判明した.そして,Hootmanら5は,National Collegiate Athletic Association(NCAA)Injury Surveillance Systemを用いて,27年間で報告された15種類の大学スポーツ競技における傷害データ調査を行った.その結果によると,NCAAでは,足関節捻挫が最も高い割合(15%,0.83/1000athleticexposures)であったことが報告された.足関節捻挫の発生率の高さやスポーツ人口における罹患率を考えると,足関節捻挫に対する社会的費用は計り知れない負担とであると考えられている1.したがって,足関節捻挫に対する傷害予防の取り組みは,世界的な課題といえる.
