【緒言】
乳がんは、世界中で女性のがんの中で最も多い。乳がんの約 5-10%は遺伝性であり、
原 因 と な る 遺 伝 子 の 生 殖 細 胞 系 列 病 原 性 バ リ ア ン ト (Germline pathogenic variants:
GPVs)に 起 因 す る こ と が 知 ら れ て い る 。 最 も よ く 知 ら れ て い る の は BRCA1 お よ び
BRCA2 の GPVs である。GPVs のある女性は、ない女性に比べ、若年で乳がんを発症
する可能性が高い。アメリカの各ガイドライン{American College of Medical Genetics
and Genomics and the Association for Molecular Pathology(ACMG/AMP)や National Society
of Genetic Counselors and National Comprehensive Cancer Network(NCCN)}では、遺伝性
乳がんの遺伝カウンセリングや遺伝子検査の受診について詳しい基準が示されおり、
適切な遺伝カウンセリングと検査は、GPVs 保有者に対する予防介入やサーベイラン
スとともに、一次および二次予防を通じてリスクと総死亡率を減少させることができ
ると期待される。従って遺伝的に乳がんリスクが高い GPVs 保有者を特定するために、
適切に GPVs の遺伝子検査を提案することが重要であると考えられている。
乳がんに対する GPVs の影響の大きさを示すエビデンスの多くは欧米からの研究報
告であり、日本人を対象とした大規模な症例対照研究は桃澤らの研究(Nat.com. 2018)
だけである。しかもこの研究の対照者はがん以外の疾患を罹患しており、がんの家族
歴のある者は除外されているため、一般集団の標本とは言い難い。また、乳がんに関
連する GPVs の保有率は民族間で異なる可能性があることから、日本人を含む東アジ
アの人々でのエビデンスの集約が必要である。また、疫学的に確立された乳がんリス
ク因子を GPVs と包括的に評価した研究は乏しい。
以上を踏まえ、日本人女性における乳がんの原因となる 9 遺伝子の GPVs と乳がん
リスクとの関連について、疫学的に乳がんのリスクとして確立された環境因子や遺伝
子多型を共変量として考慮した症例対照研究を実施した。
【方法と結果】
2001 年 1 月から 2005 年 12 月まで実施された愛知県がんセンターの病院疫学研究プ
ログラム(HERPACC2)の女性参加者の中から、乳がん症例と、乳がん症例と年齢と閉
経状況をマッチさせた対照者(がんの既往歴のない者)を 1:2 の割合でピックアップし、
最終的にデータが得られた 625 名の乳がん症例と 1133 名の対照者について症例対照
研究を行った。調整因子として年齢、飲酒、喫煙量、閉経状況、BMI、運動習慣、来
院理由、遺伝子多型によるリスクスコア(Sueta et.al. BCRT 2012)を用いた。
上記研究対象者の genomic DNA を用いて、9 遺伝子の GPVs について次世代シーク
エンサーを用いて探索したところ、25 か所の GPVs を同定した(ATM:2 か所、BRCA1:
6 か所、BRCA2:13 か所、CDH1:0、CHEK2:1 か所、PALB2:1 か所、PTEN:1 か所、
STK11:0、TP53:1 か所)。
GPVs 保有割合を見てみると、乳がん症例では 625 名中 25 名で 4.0%、対照者では
1133 名中 4 名で 0.4%であった。遺伝子別では BRCA1 と BRCA2 のいずれか、または
両方の GPVs が最も多かった(症例 19 名・3.0%、対照 2 名・0.2%)。また乳がん症例の

年齢層別での GPVs の保有率は 40 歳未満で最も割合が高く(6.7%)、統計学的には有
意ではないものの年齢が上がるにつれて減っていく傾向であった(P = 1.5E-01, Figure 1)。
GPVs 保有症例と非保有症例の平均年齢はそれぞれ 48 歳と 52 歳で、保有症例は統計
学的に有意に若年で乳がんと診断されていた(P = 3.6E-03)。
次に GPVs と乳がんとの関連を評価したところ、GPVs 保有者は非保有者に比べ乳
がんリスクが有意に高く 12.2 倍であった(P = 1.7E-06)。また年齢別に見てみると、60
歳未満で GPVs 保有者は非保有者に比べ乳がんリスクが確実に高かった{40 歳未満:
23.5 倍(P = 3.7E-02)、40-49 歳:31.3 倍(P = 2.1E-02)、50-59 歳:6.5 倍(P = 1.1E-02)、
Table 1}。対象遺伝子を BRCA1 と BRCA2 遺伝子に絞った場合でも同様の結果であった
{40 歳未満:20.5 倍(P = 4.9E-02)、40-49 歳:21.8 倍(P = 4.0E-02)、50-59 歳:4.8 倍(P
= 4.3E-02)}。
遺伝子別に GPVs と乳がんリスクの関連を解析したところ、BRCA1 と BRCA2 のど
ちらかまたは両方に GPVs がある人は 16.0 倍(P = 5.0E-02)、BRCA1 だけでは 29.6 倍
(P = 2.2E-02)、BRCA2 だけでは 11.0 倍(P = 6.6E-04)という結果で有意にリスクが上が
っていた。GPVs のあった ATM、CHEK2、PALB2、PTEN、TP53 についても、統計学的
に有意では無いもののリスクが上がる傾向にあった(Table 2)。
さらに遺伝子変異のタイプ別に解析を行ったところ、機能喪失を引き起こす遺伝子
変異において乳がんリスクが有意に高くなっていた{stop gain/loss 変異(14.9 倍、P =
2.1E-02)、frame shift short in/del 変異(5.0 倍、P = 1.7E-04)、splicing site 変異(1.4 倍、
P = 2.1E-02)、Table 3}。
これらの GPVs と乳がんリスクの評価は、疫学的に確立された環境因子や遺伝子多
型を共変量として考慮した解析で行なった。Figure 2 に示すように遺伝子多型による
リスクスコアと GPVs には若干の交絡が生じるものと推測されたが、環境因子の間に
明らかな交絡は認められず、GPVs と乳がんリスクとの関連は十分に強いものと認め
られた。
【考察・結語】
日本人女性の集団を対象とした、本邦で初めての疫学的に適切にデザインされた症
例対照研究において、環境因子などの確立されたリスク要因を考慮した上でも GPVs
と乳がんリスクとの強い関連を確認した。本研究において、乳がん症例における GPVs
の保有者の割合が欧米の結果と比べて低かった事は、GPVs の頻度が人種間で異なる
可能性を示した。現在のアメリカ NCCN ガイドラインでは遺伝性乳がんを念頭に置い
た遺伝子検査の対象を 50 歳未満と推奨しているが、本研究において統計学的に有意
なリスク上昇が 50-59 歳でも認められた事は、日本人女性に合わせたガイドラインの
必要性を示唆する。また変異タイプ別の解析でタンパクの機能喪失を引き起こすバリ
アントは乳がんリスクが有意に上昇していた。機能や遺伝子データの不足により、現
在 Variant of uncertainly significance(VUS)として分類されている変異の存在が推測さ
れ、将来的に病原性として分類される変異が浅在することが示された。最後に日本を

含む東アジアでは GPVs が乳がん発症に与える影響の調査が十分量実施されておらず、
さらなる研究が必要である。
