【緒言】
ニューモシスチス肺炎(Pneumocystis pneumonia: PCP)は P. jirovecii 感染による日和
見感染症である。HIV 感染や免疫抑制治療などで細胞性免疫不全を起こすと病原体が
排除されず 顕性感染と なる。HIV 患者におけ る抗レトロ ウイルス療 法(antiretroviral
therapy: ART)が一般的となった現代でも、PCP は最も頻度の高いエイズ指標疾患であ
る。
HIV-PCP 患者における ART 導入時期を検討した 2 つのランダム化比較試験が報告
されているが、重症 PCP 症例は除外されており、ART 導入時期に関す臨床的コンセン
サスは未だ存在しない。また前述の 2 試験では ART として主にプロテアーゼ阻害剤
(protease inhibitor: PI)を含むレジメンが選択されているが、近年主流となっているイ
ンテグラーゼ阻害剤(integrase strand transfer inhibitor: INSTI)を含むレジメンの適用性、
実現可能性を評価する必要がある。そこで重症 PCP 患者を含むリアルワールドの HIVPCP 患者群における早期 ART 導入の有効性と安全性を評価した。
【方法】
名古屋医療センターで 2014 年 4 月から 2019 年 12 月までに HIV 感染による PCP 発
症 AIDS と診断し、初回 ART を導入した HIV-PCP 症例を抽出し、後方視的コホート
研究を行った。PCP 治療開始後 21 日以内に初回 ART を導入した早期群、22 日以降に
ART を導入した晩期群に分類した。ART 開始後 24 週時点で転帰を評価した。主要評
価項目は日和見感染の新規発症あるいは再燃による AIDS 進行と全死亡、副次評価項
目は血中ウイルス量が 50copies/mL を下回るウイルス学的制御率と CD4 数の変化とし
た 。 そ の 他 安 全 性 評 価 と し て 免 疫 再 構 築 症 候 群 (immune reconstitution inflammatory
syndrome: IRIS)の頻度、グレード 3~4 の検査値、臨床所見の有害事象を評価した。
【結果】
初回 ART 導入された AIDS 患者 133 人のうち、HIV-PCP 患者は 99 人だった。解析
不能例 8 例が除外され、早期 ART 群 36 例、晩期 ART 群 55 例を割り当てた(FIGURE 1)。
2 群間の基本特性を TABLE 1 に示す。平均年齢は早期 ART 群(46.8 歳)が晩期 ART
群(41.0 歳)より有意に高齢だった(p=0.02)。性別、人種に関して 2 群間での特性は同
等だった。35 例の呼吸不全症例、4 例の気胸合併例を認めたが、2 群間で有意差を認
めなかった。併存疾患に関しても有意差を認めなかった。
ART 導入前の検査値を TABLE 2 に示す。CD4 数中央値は早期群(26 cells/μL)、晩期
群 (17 cells/μL)で あ り 、 CD4/CD8 比 と と も に 早 期 群 の 方 が 有 意 に 高 か っ た (p<0.01,
p=0.02)。それでも大部分の症例は重度の免疫不全状態であった。ウイルス量もやや早
期群の方が高い傾向があるものの、2 群間で有意差は認めなかった。炎症性マーカー
の値も同程度だった。PCP 治療開始~ART 開始までの日数中央値は、早期群で 15 日、
晩期群で 31 日だった。
ART レジメンと ART 導入時のステロイド投与を TABLE 3 に示す。ART レジメンは

1 例を除いて INSTI 1 剤と核酸系逆転写酵素阻害薬(nucleos(-t)ide reverse transcriptase
inhibitor: NRTI)2 剤で治療された。早期群では 2017 年以降の症例がやや多かったため、
NRTI として emtricitabine(FTC)/tenofovir alafenamide(TAF)を選択した割合が多かった
(p=0.03)。早期群は全例 PCP 治療中であり、その半数がステロイドを併用しつつ ART
導入されており、晩期群より有意に割合が高かった(p<0.01)。
評価可能な症例は早期群 34 例、晩期群 54 例だった(TABLE 4)。主要評価項目であ
る ART 導入後 24 週時点での AIDS 進行/全死亡は早期群 12%、晩期群 9%で統計学的
有意差を認めなかった(p=0.73)。早期群で 1 例死亡例があったが、死因は HIV 進行や
治療とは関係なかった。副次評価項目としてウイルス学的制御率は早期群 94%、晩期
群 80%で早期群の方が多い傾向だったが有意差は認めなかった(p=0.07)。CD4 変化量
は両群でほぼ同等だった。安全性評価として IRIS は早期群 27%、晩期群 20%だった
(p=0.60)。PCP-IRIS の頻度は早期群 12%、晩期群 9%で同等だった(p=0.73)。PCP-IRIS
は前例軽症であり、ステロイド増量により軽快した。グレード 3~4 の検査値、臨床所
見の有害事象もほぼ同等だった(p=0.60, p=1.00)。
両群の基本特性は特に年齢、ART 開始前の CD4 数に有意差を認めたため、今回は年
齢、性別、人種、呼吸不全、気胸、ART 導入前の CD4 数、ウイルス量に関してプロペ
ンシティスコアマッチングを行い、基本特性のそろった 24 人ずつのコホートを選定
した。基本特性のそろった 2 群で改めて主要評価項目、副次評価項目、安全性評価を
検討した。結果はいずれの項目に関しても両群で有意差を認めなかった(TABLE 5)。
以上より HIV-PCP 患者に対する早期 ART 導入は忍容性が高く安全であることが示さ
れた。
【考察】
HIV-PCP 患者における ART 導入時期を検討した 2 つのランダム化比較試験が報告
されている。ACTG A5164 試験では日和見感染治療開始 14 日以内に ART 導入した群
では日和見感染症治療が終了した後に ART 導入した群と比較して、エイズ進行/全死
亡の頻度が低く、有害事象の増加がないことが示された。しかし全体のおよそ 60%を
占める HIV-PCP のサブグループに限定すると、統計的優位性を示すことができなかっ
た。加えて呼吸不全症例の割合が不明だった。ART レジメンは 85%以上の症例でブー
ストした PI 1 剤と NRTI 2 剤が選択された。
IDEAL 試験では日和見感染治療開始 7 日以内に ART 導入した群では日和見感染症
治療が終了した後に ART 導入した群と比較して、病勢進行や IRIS、免疫学的あるい
はウイルス学的転帰、生活の質に負の影響がなく安全だったことが示された。しかし
HIV-PCP は早期群 24 例、晩期群 26 例に限られており、重篤な有害事象の発生率のわ
ずかな差は検出できなかった可能性がある。また重症例が除外されたかどうかに関す
る 記 載 が な か っ た 。 ART レ ジ メ ン は atazanavir/ritonavir と tenofovir disoproxil
fumarate/emtricitabine が選択された。
そこで我々は重症例を含む HIV-PCP 集団において、INSTI を含む ART レジメンを

早期に導入する有効性と安全性を検証した。結果的に ACTG A5164 試験と病勢進行に
関して異なる結果となった。ACTG A5164 試験では PCP 治療開始後 15~21 日目に
ART が導入された例はなく、患者集団が実際の臨床現場でみられるものとは異なって
いた。これに対して本研究では PCP 治療開始後 21 日以内に ART 開始した患者を早期
群と定義した点、38%が呼吸不全を伴う重篤な PCP であった点において、実臨床を反
映している。
INSTI を含む ART は IRIS の発生率の増加と関連していたという報告がある。我々
の研究ではすべての PCP-IRIS 症例は軽度であり、ステロイドの投与量を増やすこと
で回復した。以上より、PCP を有する未治療の HIV 陽性患者において、早期の ART
導入は忍容性が高く安全であると考えられた。
【結論】
PCP 発症 AIDS に対して、INSTI を含む ART を早期に導入することを積極的に検討
しうると考えられた。
