ケネディークラスI患者の偏咀嚼と咀嚼能率に関する前向き比較研究

概要

背景及び目的:Kennedy Class I患者(KCI群)における偏咀嚼については十分に定義されていないのが現状である.本研究では,KCI群の偏咀嚼と咀嚼能率について,残存臼歯の有無や可撤性義歯(RPD)治療の意義を含めて検討することを目的とした.

材料及び方法:九州大学病院補綴科を受診し,可撤性義歯(RPD)新製予定のKCI患者(KCI群)44名を対象とし,KCI群を臼歯部咬合支持に左右差がある患者22名(左右差あり群)と,差がない患者22名(左右差なし群)に分けた.また,ポジティブコントロールとして,健常有歯顎者20名(HD群)を対象とした.咀嚼時の筋活動から算出したMastication predominance index(MPI, 0-100%)で定義される偏咀嚼と,RPD治療前後の咀嚼能率を評価した.

結果:KCI左右差あり群のMPIはHD群に比べて有意に高かった.RPD治療は,両KCI群のMPIと咀嚼能率を有意に改善した.しかし,RPD治療の有無に関わらず,両KCI群とHD群の間には咀嚼能率に有意差が認められた.

結論:Kennedy Class I患者の偏咀嚼には臼歯部咬合支持数の左右差が影響している.また,可撤性義歯治療により,Kennedy Class I患者の偏咀嚼と咀嚼能率は改善されたが,咀嚼能率は健常有歯顎者に及ばなかった.
