【緒言】
現代の高齢社会においてサルコペニアは最も重要な健康問題の一つである。大腿は
加齢と筋肉に関する研究によく用いられ、その評価方法としてコンピュータ断層撮影
(CT)は有効な手段である。しかし CT を用いてサルコペニア重症度と筋肉量や筋内脂
肪量の変化をみた報告は少ない。また骨粗鬆症はサルコペニアと共通の病態生理的要
因を有しており、互いに負の方向へ影響しあっている。本研究の目的は、日本人の中
年から老年の大腿全体の筋肉、筋間脂肪、皮下脂肪面積、CT 値(CTV)とサルコペニア
重症度との関係、加えて骨粗鬆症とサルコペニアの重症度との関連について明らかに
することである。
【対象及び方法】
2016 年から 2017 年に国立長寿医療研究センター・ロコモフレイル外来を受診した
321 人(男性 116 人、女性 205 人、平均年齢 77.2±7.1 歳、年齢範囲 53~96 歳)を対象と
した。Asia Working Group of Sarcopenia 2019 に基づき、患者を正常(N)群、機能低下
(LF)群(筋肉量は正常だが筋力もしくは身体機能が低下している)、サルコペニア(S)
群、重度サルコペニア(SS)群の 4 群に分けた。大腿中央部 CT を撮影し、専用ソフト
ウェアを用いて筋肉面積、筋間脂肪面積、皮下脂肪面積、CTV を計測した。 筋肉・皮
下脂肪・筋間脂肪の面積及び面積比、筋肉の CTV、筋肉面積と CTV の積を 4 群間で
比較した。骨密度は young adult mean(YAM 値)を測定し、80%以上を正常、70%以上
80%未満を骨塩減少、70%未満を骨粗鬆症とした。4 群間での骨粗鬆症の分類の人数、
腰 椎 と 大 腿 骨 頚 部 の YAM 値 の 平 均 を 比 較 し た 。 統 計 解 析 は SPSS Statistics
ver28.0.0(IBM, Chicago, Illinois, USA)を用いて行い、p<0.05 を統計的に有意差ありと
した。
【結果】
4 群間で年齢、身長、体重、BMI を比較したところ男性の身長以外の項目において
差を認めた(Table1)。面積に関する項目の平均値とサルコペニア 4 群間の比較の結果
は、大腿全面積、筋肉面積、皮下脂肪面積は男女とも有意差を認め(大腿全面積、筋肉
面積は男女とも p<0.001、皮下脂肪面積は男性 p=0.039、女性<0.001)、筋間脂肪面積、
皮下脂肪面積/筋間脂肪面積、筋間脂肪/筋肉面積は女性のみ有意差を認めた(各
p<0.001, p=0.004, p<0.001)。大腿全面積は、男性では LF 群は N 群、S 群、SS 群より有
意に大きく(各 p=0.006, p=0.000, p=0.000)、女性では S 群、SS 群は N 群より有意に小
さく(各 p=0.001, p=0.000)、また LF 群より有意に小さかった(各 p=0.001, p=0.000)。
筋肉面積は、男女ともに S 群、SS 群は N 群、LF 群より有意に小さかった(男性:N-S
群間 p=0.002, N-SS 群間 p=0.000, LF-S 群間 p=0.000, LF-SS 群間 p=0.000, 女性:N-S 群
間 p=0.000, N-SS 群間 p=0.000, LF-S 群間 p=0.029, LF-SS 群間 p=0.000)。女性の筋間脂
肪面積は、LF 群は N 群より有意に大きく(p=0.004)、S 群、SS 群は LF 群より有意に
小さかった(各 p=0.024, p=0.015)。皮下脂肪面積は、男性では全体で有意差がある

(p=0.039)ものの各群間の比較では有意差を認めなかった。女性では、LF 群は S 群、
SS 群より有意に大きく(各 p=0.018, p=0.003)、SS 群は N 群より有意に小さかった
(p=0.017)。女性の皮下脂肪面積/筋間脂肪面積は、SS 群は N 群より有意に小さかっ
た(p=0.005)。女性の筋間脂肪面積/筋肉面積では、LF 群は N 群よりも有意に大きかっ
た(p=0.000)(Table2)。CTV とサルコペニア 4 群間の比較の結果は、男性では S 群、SS
群は N 群より有意に CTV が低く(各 p=0.001, p=0.001)、女性では N 群は LF 群、SS
群より有意に高い結果(各 p=0.000, 0.000)であった。SS 群は S 群より有意ではないも
のの有意水準に近く低値であった(p=0.068)。筋肉面積×CTV は、男女とも有意差を
認め(いずれも p<0.001)、群間比較では男性では S 群、SS 群は N 群より有意に低値で
あり(各 p =0.000, 0.000)、S 群、SS 群は LF 群より有意に低値であった(各 p=0.000,
0.000)。女性では LF 群、S 群、SS 群は N 群より有意に低値であり(各 p=0.019, 0.002,
0.000)、SS 群は LF 群より有意に低値であった(p=0.000)(Table3)。
各群の骨粗鬆症分類の内訳は、男性のサルコペニアでは骨粗鬆症罹患者が有意に多
かった(p=0.019)が、女性では関連を認めなかった(Table4)。腰椎、大腿骨頚部の YAM
値を 4 群間で比較すると、男女ともに腰椎、大腿骨頚部いずれも有意差を認めた(男
性:腰椎 p=0.008, 大腿骨頚部 p=0.008 女性:腰椎 p<0.001, 大腿骨頚部 p=0.016)。男
性は腰椎、大腿骨頚部ともに SS 群は LF 群より有意に低値であった(各 p=0.012, 0.027)。
女性は腰椎では S 群は N 群・LF 群より有意に低値であり(各 p=0.021, 0.008)、大腿
骨頚部では SS 群は N 群より有意に低値であった(p=0.018)(Table5)。
【考察】
筋肉面積は男女ともに N→LF→S 群とサルコペニアの重症度が上がると小さくなる
ことが示された。 大腿全面積、脂肪面積は男女で異なる結果となった。男性は LF 群
の大腿面積は他の群より有意に大きく、筋間脂肪や皮下脂肪面積には 4 群間で差を認
めなかった。女性では S、SS 群は他の2群より全面積と皮下脂肪面積が有意に小さか
った。このことから男性のサルコペニアは筋肉量の減少に起因し、女性のサルコペニ
アは筋肉と皮下脂肪の減少が影響すると考えられる。
女性の LF 群は他の 3 群より有意に筋間脂肪面積が大きかった。また LF 群は CTV
が N 群より有意に低く、S、SS 群より筋肉面積が大きかった。筋肉面積×CTV も SS
群より高値であり、言い換えると筋内脂肪の割合も高い。有意差はないものの N 群よ
り皮下脂肪も多い。これらより LF 群は筋肉量は落ちていないものの脂肪の多い肥満
状態となっていると考えられる。今回、筋肉と脂肪の分布を詳細に検討したことで LF
群の特徴を捉えることができた。
骨粗鬆症とサルコペニアの重症度については、男性はサルコペニアになると骨粗鬆
症患者が増加していた。各群での平均 YAM 値は S、SS 群は N,LF 群より有意に低く、
特に男性では LF 群と SS 群間で腰椎、大腿骨頚部共に有意差を認めた。女性では腰椎
で N 群と S 群間と LF 群と S 群間に、大腿骨頚部で N 群と SS 群間に有意差を認めた。
サルコペニアで BMD 低値、骨粗鬆症有病率が高いという報告が散見されており、筋

肉量の減少は骨の健康に重大な影響を及ぼすことが示唆される。
【結語】
日本人の中年、老年において、筋肉量だけでなく脂肪量や CTV も併せることでサル
コペニアの病態を検討することができ、男女間の差も示すことができた。さらに機能
低下群の病態についても明らかにできた。また男性においてサルコペニアと骨粗鬆症
の関連を認めた。
