【緒言】
腰部脊柱管狭窄症(LCS)は退行性変化により脊柱管の狭窄が生じ、馬尾と神経根の
圧迫を来す疾患であり、その有病率は一般人口の 10%にのぼると報告されている。歴
史的に腰椎後方除圧術は広範な椎弓切除から始まったが、正常構造を破壊することに
よる変性の進行が明らかとなり、不安定性の惹起も懸念されるようになった。そのた
め様々な低侵襲手術が開発されており、現在では figure1 に示す棘突起縦割式椎弓切除
術(LSPSL)や figure2 のような片側進入両側除圧術(ULBD)が広く行われている。我々
は従来、LCS に対して全例に LSPSL を行なっていたが、経過観察中に変性の進行が原
因で再発する症例を複数経験したため、これら変性の進行が後方正中構造や椎間関節
への侵襲にあるのではないかと仮定し、2017 年 5 月から全例に ULBD を行うよう術
式を変更した。
【目的】
LSPSL には広く左右対象な術野が得られるというメリットがあるが、正中構造を破
壊せざるを得ないというデメリットがある。一方、ULBD は正中構造を完全に温存で
きるという意味で非常に低侵襲な術式であるが、進入側の外側陥凹部の視野が得られ
づらいという欠点がある。この欠点に対して我々は ULBD を行う際に棘突起を若木骨
折させて対側に傾け、進入側手前側の視野を得やすくするという改良を加えた
(figure3)。これらをふまえ、同一グループが行なった LCS に対する適応を同じくした
2 種類の手術法(LSPSL と ULBD)について、どちらの術式が優れているか明らかにす
るために本研究を行った。
【方法】
2015 年 4 月から 2019 年 4 月までに LCS に対して LSPSL または ULBD を施行した
平均年齢 70.8±9.4 歳の連続 141 例について後方視的に比較検討を行った。2015 年 4 月
から 2017 年 4 月に LSPSL を受けた 73 名の L 群と、2017 年 5 月から 2019 年 4 月に
ULBD を受けた 68 名の U 群の 2 群に分け、術後 1 年間の経過観察期間における臨床
的・放射線学的転帰と手術合併症を評価した。放射線学的パラメータについては
Student t 検定を行い、Visual analog scale(VAS)と日本整形外科学会腰痛治療判定基準
(JOA score)の分析には Mann-Whitney U 検定を用いた。有意水準は P < 0.05 とした。
【結果】
2 群間で手術時間に有意差はなかったが、U 群は L 群に比べ出血量が有意に少なく、
椎間関節の温存率も高かった。また、L 群では 3 名が再発のために再手術を必要とし
たが、U 群に症候性の再発は見られなかった(table1)。両群間で術後成績やアライメン
ト、脊柱管面積拡大率について統計学的有意差は見られなかったが、L 群に対して U
群は優位に椎間関節の温存率が高かった(table2)。多椎間症例や辷り症合併症例を別々
に検討した結果、両手術は同等に有効であり、VAS と JOA score は、各群とも術後に

有意に改善した(figure4)。
【考察】
低侵襲手術の需要が高まる中、LCS に対する標準的な椎弓切除、ULBD、LSPSL、内
視鏡手術などの異なる手術方法を比較した研究は近年増えてきている。一般に、ULBD
や LSPSL などの低侵襲手術は、入院期間、術後の腰椎不安定性、出血量、創部痛の軽
減において、標準的な椎弓切除より優れているといわれている。このうち内視鏡手術
は、皮膚切開が小さく術後の回復が早いことから、近年特に注目されているが、これ
までの LCS に対する内視鏡手術の研究はほとんどが単椎間症例のみの解析であり、多
椎間症例や辷り症、脊椎不安定症に対しては言及されていない。内視鏡手術の有効性
は顕微鏡手術に匹敵するかもしれないが、多椎間症例や辷り症合併症例に対する内視
鏡手術の汎用性を分析するためにさらなる研究が必要である。我々の開発した 改良
ULBD は棘突起を傾斜させることにより進入側の外側陥凹部まで斜めにアプローチで
き、より広い術野を確保することができる。これにより、進入側の神経根を効果的に
除圧することができ、椎弓の切除範囲、関節包への侵襲、傍脊柱筋の剥離を最小限に
抑えることができる。また、棘間靭帯や棘上靭帯は従来の ULBD と同様完全に温存さ
れる。今回の検討では U 群と L 群では術後の脊椎アライメントに差は見られなかった
が、L 群では術後 1 年目に 3 名の患者に症候性の再発が起こった。この 3 例のうち、
2 例は多椎間手術(3、4 椎間)、1 例は単椎間手術であった。Bresnahan らは腰椎減圧モ
デルを用いて、後方正中構造の温存が腰椎に与える影響を検討し、後方正中構造の破
壊は腰椎の可動域を拡大し、脊椎の変性を進行させると結論付けた。また、Schär らは、
多椎間症例に対して正中構造を温存した除圧術を行なって良好な成績が得られたこと
を報告している。これらの報告は本研究で L 群のみに 3 例の再発がみられた原因が後
方正中構造への侵襲にあるということを支持しており、ULBD は LSPSL と比較して正
中構造、椎間関節を温存するという意味でより優れた術式であることが示された。
【結論】
LSPSL、ULBD はいずれも腰部脊柱管狭窄症に対する安全かつ有効な術式であり多
椎間症例や辷り症合併症例に対してさえも同様であることが明らかとなった。我々の
改良 ULBD は椎間関節や正中構造の温存という点で LSPSL よりも優れており、これ
ら組織の温存が多椎間症例における再発や変性を起こりにくくさせることが示唆され
た。
