The relationship between oral hypofunction and sarcopenia in patient with no dental complaint : A cross-sectional study

概要

目的 口腔機能とサルコペニアの関連はいくつかの先行研究が報告している。しかし、これまでの研究では口腔内の疼痛や歯周病の進行等に起因した咀嚼器官の欠損によって起こる咀嚼障害(器質性咀嚼障害)の影響はほとんど考慮されていない。本研究は一般歯科診療所の歯科的主訴の無い患者を対象に、口腔機能低下症に関する口腔機能評価とサルコペニアとの関連を調査することを目的として実施した。方法 本研究は2019年6月から2021年3月の期間に一般歯科診療所を受診した65歳以上の外来患者のうち歯科定期検診のため通院中あるいは治療終了後、定期検診へ移行した患者を対象とした横断研究である。269名の研究参加者(平均年齢74.9歳±6.50、男性133名、女性136名)に対し口腔機能精密検査、AWGS2019基準によるサルコペニアの評価を行い両者の関連を検討した。結果 研究参加者のうち口腔機能低下症と診断されたのは132名(49.07%)であった。また30名(11.2%)がsarcopenia、24名(8.9%)がsevere sarcopeniaと判定された。ポアソン回帰分析の結果、舌口唇運動機能[pa](prevalence rate ratio[PRR]=0.80、95%Wald=-0.44--0.02)、[ka](PRR=0.76、95%Wald=-0.53--0.03)、舌圧(PRR=0.95、95%Wald=-0.09--0.02)、口腔機能低下該当項目数(PRR=1.39、95%Wald=0.11-0.56)は有意にサルコペニアと関連していた。結論 器質性咀嚼障害の影響の少ない高齢者においても口腔機能低下とサルコペニアとの間に有意な関連が認められた。社会の高齢化に伴い、これからの歯科医師は器質性咀嚼障害の治療のみではなく、運動障害性咀嚼障害とサルコペニアの関連を視野にいれた治療が求められると示唆された。
