東日本大震災の被災者におけるレジリエンスの縦断変化のパターンとメンタルヘルスとの関連性

概要

レジリエンスとは、逆境を体験しても精神的な健康を維持・回復する能力を指す。横断的観察に基づいた先行研究の結果、レジリエンスと抑うつ症状等メンタルヘルス 症状を測定する質問票との間には負の相関関係が示されている。災害後のフォローア ップを行っていく上で、レジリエンスとメンタルヘルスとの関連性について縦断的な 特徴を把握しておくことは有益であると考えられるが、レジリエンス自体の変化を含 め、メンタルヘルスとの関連性を縦断的に検証した研究はこれまでほとんどなされて いない。そこで本研究は、東日本大震災で被災した成人のレジリエンスの縦断変化に ついて検証し、それらとメンタルヘルスとの関連性を明らかにすることを目的とした。

東日本大震災の発災当時宮城県七ヶ浜町に居住し、住居が大規模半壊以上の被害を受けた世帯の住民を対象に、発災年から毎年健康調査を実施した。毎年の調査においては、属性等に加え、メンタルヘルス指標として、心的外傷後ストレス反応、抑うつ症状、心理的苦痛をそれぞれ測定する、IES-R（The Impact of Event Scale-Revised）、 CES-D（The Center for Epidemiologic Studies Depression Scale）、K6（The 6-item Kessler Psychological Distress Scale）を用いた。加えて 2012 年と 2016 年の調査では、CD-RISC- 10（The 10-Item Connor-Davidson Resilience Scale）を用いてレジリエンスを測定した。本研究では、2012 年と 2016 年ともに IES-R、CES-D、K6 および CD-RISC-10 の全質問項目に完答した 406 名を分析対象とした。

2012 年と 2016 年のCD-RISC-10 得点を投入したクラスター分析により、対象者を、レジリエンスが、（1）ベースライン時点から低いまま維持した群、（2）ベースライン が中度から減少した群、（3）ベースラインが中度から増加した群、（4）ベースラインが高度から減少した群、（5）ベースラインが高度から増加した群の 5 群に分類した。レジリエンスが経時的に増加した 2 群では、IES-R、CES-D、K6 得点が有意に減少した。レジリエンスが経時的に減少した 2 群では、CES-D 得点が有意に増加した。レジリエンスが低いまま維持した群では、CES-D と K6 の平均得点がカットオフポイント以上を維持した。これら 5 群を従属変数とした多項ロジスティック回帰分析の結果、レジリエンスの経時的な減少に関連するリスク要因として、男性であること、1 人か 2 人暮らしであること、またベースラインのレジリエンスが低いことに関連するリスク要因として、70 代以上の高齢者であること、経済状況が苦しいことが検出された。

発災から年数が経過し社会の復興が進む中、成人のレジリエンスは、経時的に増加した集団がある一方で、減少した集団、低いまま維持した集団があることを示した。レジリエンスが経時的に減少した集団では抑うつ症状が悪化し、レジリエンスが低いまま維持した集団では、抑うつ症状と心理的苦痛がハイリスク状態を維持した。自然災害発生後の被災者の心理社会的評価に際してレジリエンスを評価することは有用であり、災害後のフォローアップを行う上で、レジリエンスにも注意していく必要性、中でもレジリエンスが経時的に減少する人たち（特に、男性、1 人か 2 人暮らしの人）や、もともとレジリエンスが低い人たち（特に、70 代以上の高齢者、経済状況が苦しいと感じている人）に、より注意が必要であることが示唆された。
