日本における開放性眼外傷の疫学的研究

概要

人は情報の8割を視覚から得ていると言われており,視覚障害はQuality of Lifeに大きな影響を及ぼす.本邦における視覚障害の原因は緑内障,網膜色素変性,糖尿病網膜症であるが,眼外傷も社会的失明に至る眼疾患の一つである.眼外傷は損傷の程度により開放性眼外傷と非開放性眼外傷に分類される.眼科手術の進歩により眼疾患の視力予後は改善してきているが,開放性眼外傷は手術が発達した今でも視力予後不良である.近年のビッグデータ解析などにより多くの疾患で臨床的特徴が解明されてきているが,開放性眼外傷は稀な疾患であるため,単一施設では臨床的特徴を把握するには困難である.そこで我々は本邦の中核病院10施設で症例データを共有し症例数を蓄積することで,本邦における開放性眼外傷の統計や特徴を解析した.本論文では,本邦における開放性眼外傷の特徴を述べるとともに,開放性眼外傷において一般的な受傷機転である,転倒,就労,スポーツ,および交通事故を中心に述べ,さらに視力予後不良と思われる初診時に光覚弁なしの症例について説明する.
