子宮全摘出術を受けた子宮がん患者と配偶者とのコミュニケーションの特徴—子宮がん患者の体験に伴う感情に焦点をあてて—

概要

目的：子宮全摘出術を受けた子宮がん患者（以後，子宮がん患者）と配偶者とのコミュニケーションの特徴を，子宮がん患者の体験に伴う感情に焦点をあて，明らかにすることである．
方法：子宮がん患者12名を対象に半構成的面接を行い，データは質的帰納的に分析した．真実性を確保するため，語られた内容を研究者の言葉で要約し，相違がないか子宮がん患者に確認した．本研究は，香川大学医学部倫理委員会及び香川労災病院の倫理審査を受け，承認を得て行った．
結果：子宮がん患者の体験に伴う感情と配偶者とのコミュニケーションには２つのテーマがあった．テーマ１【ネガティブな感情を感じていても，配偶者からの愛により，ポジティブな感情が生まれ，コミュニケーションをとる】は，２つのサブテーマから構成されていた．テーマ２【ネガティブな感情を感じていても，配偶者の特性や子宮がん患者の認識により，コミュニケーションを制限する】は，３つのサブテーマから構成されていた．
考察：子宮がん患者は，配偶者とコミュニケーションをとる，または制限することで，ネガティブな感情をコントロールし，子宮がん罹患により引き起こされる様々な心理的課題の適応に向かっていると考えられる．配偶者からの愛は子宮がん患者の適応力を高めるが，子宮がん患者の不安を増強させる配偶者の性格や態度，配偶者には女性のことを理解し難いという子宮がん患者の認識は適応力を弱めると考えられる．
結論：子宮がん患者の適応力を高めるため，配偶者が子宮がん患者を支援できている時は，夫婦の時間がとれるよう環境調整が必要である．そうでない時は，配偶者に子宮がんに関する知識の提供や，細やかに配偶者を観察し，積極的に声かけし相談を受ける機会を設けることが必要である．さらに，子宮がん患者の認識が変わるよう介入することも必要である．
The purpose of this study was to identify the characteristics of communication between uterine cancer patients who underwent total hysterectomy and their spouses, by focusing on emotions associated with the experience of such patients. Semi-structured interviews were conducted involving 12 uterine cancer patients, and the obtained data were analyzed using a qualitative and inductive approach. To ensure authenticity, narratives summarized by the researchers were checked by the patients. This study was conducted with the approval of the ethics committee of the Faculty of Medicine, Kagawa University, and Kagawa Rosai Hospital.
Communication between uterine cancer patients and their spouses was summarized into 2 themes: [experiencing negative emotions, but developing positive emotions when feeling loved by the spouse, and actively communicating with him], [experiencing negative emotions, but limiting communication with the spouse based on his characteristics and the patient's own recognition].
To adapt to various psychological problems due to the development of the disease, the uterine cancer patients may have controlled their negative emotions by actively communicating with their spouses or limiting such communication. The results indicate that these patients' adaptability increases when they feel loved by their spouses, and decreases when their anxiety intensifies due to their spouses' personalities or attitudes. Therefore, it is necessary for nurses to assess communication between members of each couple from their initial outpatient consultation, and support both of them, in order to promote uterine cancer patients' psychological adaptation.
